Screening Forms:
All children’s ministry workers who come into direct contact with children must fill out a Children/Youth Worker Screening/Application Form (click here), receive & sign an acknowledgement of the Procedures for Accidental Injury & Sexual Misconduct Allegations (click here), submit an online background check (click here), and read our child safety policy (click here). before working with children/youth.
